The clinician assigns the appropriate service code after the encounter. Patients do not select a billing code or visit level.
New-patient office evaluation
Your first professional office visit with the practice.

How the visit level is determined
The clinician determines the code after the visit based on documented medical decision-making or, when applicable, total physician or qualified health professional time on the date of service. The listed levels are not appointment-length choices and are not selected in advance by the patient.
What is included
The professional evaluation and management service furnished and billed by Merrimack Thoracic & Esophageal Surgery for the applicable office visit.
What is billed separately
Hospital or facility fees, procedures, anesthesia, imaging, laboratory services, pathology, medications, and services from other clinicians or organizations are not included. Separate estimates may be required.
